• I understand that massage therapists do not diagnose injuries, illnesses, or medical conditions.
   

• I acknowledge the importance of sharing any medical conditions or health concerns that may affect my care or the therapist’s well-being.
   

• I am aware that massage therapy may carry some risks, such as temporary muscle soreness, light bruising, or the aggravation of an existing or undiscovered condition.
   

• I release the company and the massage therapist from liability for any such occurrences during my session.
   

• I understand it is my responsibility to communicate any discomfort I may feel so that adjustments can be made for my comfort and safety.
   

• I respect that the massage therapist may end the session at any time if necessary. I also understand that any inappropriate behavior will result in immediate termination of the session and may lead to further action.
   

• I have read and agree to the above, and I am looking forward to this time of relaxation and self-care.
